Proposal Format
The Proposer is responsible for reading the entire attached Request for Proposal document to ensure required contents and formatting of the proposal comply with the requirements of this RFP.

All Proposals must be submitted in hard copy form to:

Proposal format

1. The entire Proposal shall be submitted in a three ring binder.
2. The binder must contain five (5) sections in the following order
a) Section A - Qualifications
b) Section B - Service Plan
c) Section C - Fee Proposal
d) Section D – References
e) Section E - Required Forms



The Proposal must be delivered in a sealed envelope or box with "Green Physical Needs Assessment Proposal 2018-1-1.” clearly marked on the outside.


The Agency intends to retain the successful proposer pursuant to a "Best Value" basis, not a "Low Bid" basis. Therefore, so that the Agency can properly evaluate the offers received, all proposals submitted in response to this RFP must be formatted in accordance with the sequence noted below. Each category must be separated by index dividers numbered (which number extends so that each tab can be located without opening the proposal). None of the proposed services may conflict with any requirement the Agency has published herein or will issue by addendum.
Tab 1: A. Qualifications
I. Summary profiles of the firm’s principals, staff, and associates, including any required certifications
II. Provide written evidence of the firm’s ability to perform the services.
III. Proposer must provide a certification statement that the firm is not debarred, suspended, or otherwise prohibited from professional practice by any federal, state, or local agency.
IV. All proposers must carry the following insurance policies: required workmen's compensation, general liability, and professional liability of no less than $1,000,000.

I. Detailed description of how proposed services as stated in this RFP will be provided
II. Listing of the deliverables the Housing Authority will receive.
III. Ability to meet the requirements of Section IV. Deliverables and Schedule

In addition to all other submissions required by this RFP, the Proposer must complete and submit as part of their submission the required forms attached to this RFP as Appendices. Those documents are self-explanatory.
Proposal Evaluation Factors
The following Factors will be utilized by the Agency to evaluate each proposal submitted.
No. 1: Proposed Cost
Maximum Point Value = 15
The proposed COST of the services.
No. 2: Experience in performing services
Maximum Point Value = 20
Verifiable and successful experience in performing Green Physical Needs Assessments and Energy Audits with blower door (air infiltration) testing for public housing agencies.
No. 3: Service Plan
Maximum Point Value = 20
Approach and experience in conducting green physical needs assessment and energy audits.
No. 4: Firm and staff qualifications/experience
Maximum Point Value = 20
Description of organization's staff and experience in HUD capital improvement programs, and facility inspections, cost estimating, green, physical needs assessments and energy audit projects, and copies of required certifications.
No. 5: Timing
Maximum Point Value = 15
Proposers shall describe start and schedule of activities to be performed.
No. 6: Overall responsiveness to RFP
Maximum Point Value = 10
Thoroughness in meeting the requirements of supplying the information requested in the RFP.
No. 7: Insurance
Maximum Point Value = 5
