Bid Submittal Format

All bidders must submit one (1) copy of the documentation / information detailed within Step #1 of Table No. 3 in the IFB 1.0 document (Tabs 1-4). Those bid submittal documents, also outlined below, are to be delivered in a sealed package and properly addressed before the bid submittal deadline.

Then, the Agency anticipates that it will notify the apparent low bidder to submit, within 5 days after being notified to do so, the information detailed within Step #2 of Table No. 3 in the IFB 1.0 document.

So that the Agency can properly evaluate the offers received, all such hard-copy bid submittals must be formatted in accordance with the sequence noted below and within Step #1 and #2 of Table No. 3 in the IFB 1.0 document. Each category must be separated by index dividers numbered (which number extends so that each tab can be located without opening the bid). None of the proposed services may conflict with any requirement the Agency has published herein or will issue by addendum.

Tab 1: Form of Bid.
This Form is attached hereto as Attachment A to this IFB document. This 2-page Form must be fully completed, executed where provided thereon and submitted under this tab as a part of the initial bid submittal.

Tab 2: Form HUD-5369-A (11/92), Representations, Certifications, and Statements of Bidders, Public and Indian Housing Programs.
This Form is attached hereto as Attachment B to this IFB document. This 4-page Form must be fully completed, executed where provided thereon and submitted under this tab as a part of the initial bid submittal.

Tab 3: Bid Bond.
The required Bid Bond as detailed within the following Section 3.4.1 herein.

Tab 4: Section 3 Business Preference Documentation (Optional Item).
For any bidder claiming a Section 3 Business Preference, he/she shall under this tab include the fully completed and executed Section 3 Business Preference Certification Form attached hereto as Attachment D and any documentation required by that form.

Lot # 2 - Deductive Alternates

Specification: Deductive Alternate No. 1: The cost of the drain in the south east area adjacent to the new dumpster pad. Include drain box, pipe, grease trap and all items associated with the box per the general conditions and instructions to bidders. See civil drawing for details and information on the drain. NOTE THESE ENTRY INSTRUCTIONS: Since this Alternate is deductive in nature, the bidder shall enter the amount with a minus sign ("-") in front of the amount entered. For example (this is an example only--the bidder must enter its own calculated amount for each deductive alternate!), as an example, if the amount of this deductive alternate is $10,000, it shall be entered as "-10,000". Failure on the part of the bidder to place the minus sign in front of this deductive amount will result in the bidder adding the amount to the base bid, not subtracting it as required; however, if a bidder fails to enter the minus sign in front of the amount as instructed, the bidders amount shall stand as a additive alternate, which will most certainly cause the bidder to lose the bid (remember, the Agency CANNOT allow a bidder to revise its bid amounts after the bid deadline, so we strongly recommend that each bidder print the receipt after entering and submitting all amounts, check those amounts for accuracy and compliance with these instructions, and, if necessary, re-enter the eProcurement System and revise any bided amounts to comply with these instructions. No Alternate Specification Allowed. Reason: NOT APPLICABLE for this item.
