Bid Submittal Format
MPHA intends to retain a contractor pursuant to a low quote basis, also taking responsiveness and responsibility into consideration. Therefore, all quotes submitted in response to this QSP must include the following completed documents and information noted below so that MPHA can properly evaluate the offers received. The quote documents and proposed fees shall be submitted online in the Housing Agency Marketplace. None of the proposed services may conflict with any requirement that MPHA has published herein or issued by addendum.
Tab 1: Section 3 Business Self Certification Form
As detailed within the QSP, any quoter may claim a Section 3 business preference by completing the Self Certification Form. Prior to award, MPHA will request additional documentation from the quoter to confirm their Section 3 status.
Tab 2: W/MBE and Section 3 Subcontractor Certification
Quoters shall document their efforts to meet the W/MBE and Section 3 participation goals by completing this certification form and submitting it with their quote submittal.
